Do not submit symptoms, diagnoses, lab results, medications, insurance details, medical history, photos, or urgent clinical concerns through this website, standard email, or any public contact form.

Patients may have rights to request access to records, request amendments, request restrictions, request confidential communications, receive an accounting of certain disclosures, and receive a copy of the practice Notice of Privacy Practices.

Those requests should be made through Spruce or the secure patient communication process provided by Longitude Wellness, not through the public website.

When a person becomes a patient, protected health information may be used or disclosed for treatment, payment, healthcare operations, care coordination, legal and regulatory requirements, public health activities, and other uses permitted or required by law.

Uses outside those purposes may require written authorization unless another legal basis applies. This public website should not be used to request or transmit those details.

Longitude Wellness should use HIPAA-appropriate vendors, access controls, signed business associate agreements where required, secure email or patient messaging, minimum necessary access, and role-based administrative controls for systems that handle protected health information.

Spruce is the intended secure communication layer for calls, SMS notifications, secure patient messages, reminders, and care-team triage once account registration, business associate terms, staff roles, and patient-facing notices are confirmed.

The public website intentionally keeps PHI out of scope while those clinical systems are configured, tested, and reviewed.
