What this website does and does not collect.
This public website does not ask visitors to create website accounts, does not provide a patient portal, does not process payments, and does not intentionally collect protected health information. The waitlist form collects only a name, email address, optional phone number, consent choice, and submission time.
Waitlist information is transmitted to Airtable and used only to administer the Longitude Wellness launch waitlist and related practice updates. It is not a medical record. Do not include symptoms, diagnoses, medications, or other private health details.
When you click a phone link, your phone provider handles the call. When you click an email link, your email provider handles the message. When the Spruce secure request link is activated, Spruce will handle secure patient messaging and related communication outside this public website.
Hosting providers may maintain standard operational logs such as request time, IP address, browser type, requested URL, and error information. These logs are used for security, uptime, troubleshooting, and abuse prevention. They are not used as a medical record.
Use of Information
How basic contact information may be used.
Basic contact details may be used to return a call, respond to a general inquiry, schedule an appointment, provide practice information, or route a visitor to Spruce or another approved secure workflow.
Longitude Wellness does not sell website visitor information. This website is not designed for behavioral advertising or retargeting.
Security Notice
Do not use this site for urgent or private medical needs.
No public website can guarantee privacy for information you voluntarily send by standard email, ordinary text, or a non-secure form. Use this website only for basic business contact.
For emergencies, call 911 or seek urgent medical care. Established patients should use Spruce or the secure communication process provided by the practice.
